By choosing this option, I hereby authorize the AFSCME Retiree Chapter 75 to charge the credit or debit card submitted with this electronic application an amount equal to the Membership dues amount specified above which shall be charged annually beginning on the date on which the application has been submitted.
The Membership dues amount cited above may be adjusted from time to time in accordance with the AFSCME International or Chapter constitution, and I will be notified in advance by writing of any such change. If the purpose of such withdrawals is restricted in any manner, such restriction is stated below. Adjusting entries to correct errors is also authorized. This authorization is effective on the date signed above and will remain in effect until written notice of termination is given to the Chapter.
Option 2 :
If you prefer to send a written check, that is great also.You will receive a letter each year reminding you to send in your check for yearly dues by July.
